Thickness of the nail. Small 'nails' like 23G are referred to as pins; next size up like 18G referred to as brads; and nails when you get up to the hefty framers.

Pin nails generally are 23 gauge and use a headless wire pin, although I have a 21 gauge Cadex gun that I call a pin nailer. It uses headless or "very slightly headed" pins. I use it for larger prefinished kitchen Crown where I can count on the pin holding better.
A Brad nailer 18ga. is useful for smaller moldings like shoe molding or the edge of window and door casing where the eye is drawn to the edge and a larger nail hole would be more obvious.
Hope this helps.
